The Indiana State Department of Health requires the following additional immunizations for any student entering the 6th grade. These immunizations are not required for 5^th grade. Please make sure the school nurse gets a record of your child having received these 6^th grade vaccines.

·         One meningococcal vaccine (MCV4) for all students entering 6th.

·         One Tdap booster One after 10 years of age for all students entering 6th.

·         Two doses of varicella (chickenpox) if child has NOT had the disease.
