5th graders be prepared for 6th grade!!
The Indiana State Department of Health requires the following additional immunizations for any student entering the 6th grade. These immunizations are not required for 5th grade. Please make sure the school nurse gets a record of your child having received these 6th grade vaccines.
· One meningococcal vaccine (MCV4) for all students entering 6th.
· One Tdap booster One after 10 years of age for all students entering 6th.
· Two doses of varicella (chickenpox) if child has NOT had the disease.
